• Prioritize them in order of preference
• For limited liability companies we suggest you include LLC at the end of the name.  (this is not mandatory).
• For corporations we suggest you include Corp. or Inc. at the endo of the name. (this is not mandatory).

• Provide your full name
• Date of birth
• Home residence
• Physical address of business or registered agent who will receive business mail
• Number of owners (members / shareholders) If multiple, all must provide information outlined above.
